Huffington Post identifies the plane as a National Airlines Boeing 747 taking off from Bagram Airfield military base.
According to NYC Aviation, it was a cargo plane operated by US Mobility Command, and all those aboard were members of the crew. No passengers were on the plane when it fell to the ground.
