An investigation into the cause of the Asiana Airlines flight crashing at San Francisco International Airport on Saturday has revealed that the pilot was having trouble before the landing.

According to USA Today, pilot Lee Gang Guk felt a light blind him when he was flying 500 feet (152 meters) above the ground. As I mentioned, this was his first time flying a Boeing 777 and he relied heavily on auto-controls.

Chairwoman of the National Transportation Safety Board, Deborah Hersman, added that the passengers were not evacuated in the first 90 seconds after the crash because the doors and slides were not open.

Flight regulations clearly state that a functional aircraft has to be evacuated within 90 seconds.

"Obviously the captain is responsible, and in this case it's the instructor in the right seat who is responsible," claims Kees Rietsema, dean at Embry-Riddle Aeronautical University.

The pilot's flight instructor was also his co-pilot and he was sitting right next to him.
