The video goes a long way back: it's a compilation of funny movie mistakes, comprised by ReelzChannel.

From the Stormtrooper banging his head on the door to the camera guy seen on the pirates' ship in “Pirates of the Caribbean” and the crew staffer in “Gladiator,” it includes flaws in some of the most recent releases, which escaped even the director's notice in the editing room.

That's not to say that the films are any less good because of them. Sometimes though, even the most awesome and near-perfect things can be flawed.
