They just killed Cash. Now, they want to kill him again. America is full of run down, broken rust-belt towns where nobody cares and anything goes. In Carcer City, nothing matters anymore and all that's left are cheap thrills. The ultimate rush is the power to grant life and take it away, for sport.

The story revolves around a man on Death Row named James Earl Cash, sentenced to death by lethal injection due to a grievous crime, the nature of which is never explained (whether Cash is truly guilty or not is also never stated). An exceedingly wealthy former Hollywood director named Starkweather, who runs a seedy community in an impoverished town named Carcer City, bribes the doctors to inject Cash with a powerful sedative instead and then bring his "dead" body to Carcer City. Brawl Game mini-game - Successfully complete levels 6 through 10 with a three star or greater rank.

Hard as Nails mini-game - Successfully complete levels 1 through 5 with a three star or greater rank.

Monkey See, Monkey Die! Mini-game - Successfully complete levels 11 through 15 with a three star or greater rank.

Time 2 Die mini-game - Successfully complete levels 16 through 20 with a three star or greater rank.
