14 DAY TRIAL // A questionnaire is a type of statistical survey usually related to a specific demographic group to gather information in order to provider better service or goods.
Questionnaires are sharply limited by the fact that respondents must be able to read the questions and respond to them. Thus, for some demographic groups conducting a survey by questionnaire may not be practical.
Questionnaires have advantages over some other types of surveys in that they are cheap, do not require as much effort from the questioner as verbal or telephone surveys and often have standardized answers that make it simple to compile data. But the standardized answers frustrate users.
phpQuestionnaire comes complete with a PHP web-based installation package. Installation is very easy and automatic: just upload the files on your server and after that start the installation. You must fill in the correct MySQL configuration variables. If you are unsure of any of the MySQL configuration parameters, please consult your web hosting provider. If you are upgrading an existing set of phpQuestionnnaire tables to the newer version, you will also need to supply the Admin Password to prove that you are authorized to make this change. This password will be the same one you have been using to login to the phpQuestionnaire admin control panel.
Each survey you create with phpQuestionnnaire can be made up of any number of questions and answers, using varying styles for each. The supported question types include: radio buttons, check boxes, select boxes (single and multiple), single line text inputs and full size text areas. Every aspect of survey creation and administration is done through a single web-based interface. This interface includes both Basic and Advanced settings so you decide how much time you want to spend configuring the look and feel of your survey versus getting it done.
When creating your surveys, questions and answers with our web-based administration program you will be able to easily preview the progress at any stage. phpQuestionnnaire allows you to preview an individual survey complete with any error messages generated from uncompleted survey questions. Our question editor allows you to preview any single question within your survey, to make sure it appears as you desire.
You may be interested in more than just the results for a specific user, however and that is why phpQuestionnnaire also allows you to see the results for all users on a specific question and excels when it comes to supplying you with the information a user has entered. You can view the results of one question at a time, one survey page at a time, or even the whole thing at once.
Your survey results can now be downloaded in several different formats. They included the CSV (Comma Separated Values) format, which is widely recognized by many different applications. Using this, you can import into Excel or another spreadsheet application and view or manipulate the data in any way you choose. We also have included the ability for you to download your results in the following additional file formats: XML (eXtensible Markup Language), XML Compressed, SQL (Structure Query Language) and Excel cell chart (for graphing purposes).
phpQuestionnnaire also includes the ability for you to pass information about your respondents to your survey and have this information appear alongside their responses.
